Is it fungus or not??? Please HELP!

I had toenail fungus all through high school and college before finally clearing it up with medication - first Diflucan then Lamisil). It came back a few years ago (I'm now 38), and after looking at it, my dermatologist put me on Lamisil. I took it on and off but decided to stop taking it since the fungus wasn't that bad.Now I'm at a new derm. A year ago he looked at it, took a sample of the debris and observed it under a microscope, and told me it wasn't fungus. Gave ma a topical laquer to put on it, but again, it wasn't too unsightly so I used it for a few days then quit.Now it's a year later, and my toe is a little worse. Also, I will wake up some nights and my big toe will be throbbing and painful, and a little red around the skin next to the cuticle. He again looked at the debris under the microscope and said it wasn't fungus. But - he told me to take Lamisil for a month and soak it in a bleach solution once in a while, then come back and let him take a look.Here is my question...could it be fungus even though he says it's not? Also, I noticed this morning a tiny black spot under the nail, and now I'm literally freaking out that there is a melanoma under there. Please someone help me feel better... I'm supposed to go back to him this week but I'm feeling like I need to go to someone else (although he is a very good doctor).
